What Is a Prior Authorization Denial?
Prior authorization (PA) — also called pre-authorization or pre-certification — is your insurer’s requirement that certain treatments be approved before they are performed. When your PA request is denied, it means your insurer decided the proposed treatment doesn’t meet its internal approval criteria.
PA denials affect an estimated 35 million patients per year. Denial rates have increased significantly since insurers adopted algorithmic review systems that apply administrative criteria without a physician reviewing the individual patient’s clinical record.

Why Prior Authorization Requests Are Denied
1. Step therapy / fail-first requirements
The insurer requires you to try and fail less expensive treatments first. Your appeal must document that you’ve completed the required steps — or argue that completing them would cause clinical harm.
2. Not medically necessary under plan criteria
The insurer’s criteria don’t align with your physician’s assessment. Your appeal must present the clinical evidence and demonstrate it satisfies the insurer’s own criteria — or that those criteria conflict with established guidelines.
3. Missing or insufficient documentation
Often the easiest to overturn: supplement the record with the missing documentation and resubmit as a formal appeal.
The Winning Prior Authorization Appeal Strategy
Argument 1: Clinical necessity is established and documented
Present objective clinical evidence: diagnosis with ICD-10 code, documented symptoms and severity, test results, and your physician’s clinical assessment. Specific documented clinical facts — not general statements of necessity.
Argument 2: Step therapy requirements have been satisfied
List every prior treatment with specific dates, dosages, duration, and documented outcomes. Show the progression explicitly: “Treatment A was tried from [date] to [date] and failed because [documented reason].”
Argument 3: Clinical guidelines support the requested treatment
Identify the professional society guideline recommending your treatment for your diagnosis — ADA, ACC/AHA, NCCN, AAD, AAOS, or AAN. A specific citation creates an accountability standard that administrative criteria cannot override without explicit justification.
Argument 4: Request peer-to-peer review
Explicitly request that your treating physician speak directly with the insurer’s reviewing physician. Studies show overturn rates exceeding 70% when peer-to-peer review occurs. State this request clearly in your letter.
